I fully and unconditionally accept the terms of the Offer (public offer) on the conclusion of an agreement on the provision of services by the Service Provider

I give informed voluntary consent to intervention on health issues in order for me (the person of whom I am a representative) to receive written or oral consultation, which is of a recommendatory nature, in accordance with the above agreements (link to the full text of the Agreement - https://mbr.fyi/consent)
