I fully and unconditionally accept the terms of the Offer (public offer) on the conclusion of a contract for the provision of medical services by the Provider of medical services

I give informed voluntary consent to the health intervention for me (the person I represent) to receive written or oral advice, which is of a recommendatory nature, in accordance with the above agreements (link to the full text of the Agreement - https://mbr.fyi/consent )

I agree with the MedBrama Service Privacy and Personal Data Policy (link to the text of the Policy - https://mbr.fyi/policy)
